As a consumer of health services, you will find it important to understand what your responsibilities are when receiving services from MonashLink Community Health Service.

Your Responsibilities

  • Provide, to the best of your knowledge, relevant and accurate information that will assist your care or treatment;
  • Participate in your agreed health care or treatment plan;
  • Ask questions of relevant staff so you can make informed decisions about your health care;
  • Keep scheduled appointments, however in the case of cancellation, provide timely notification;
  • Respect the privacy, needs and dignity of others by being courteous and considerate;
  • Observe safety features while participating in MonashLink services / programs;
  • Show consideration to staff by providing a safe environment if receiving a domiciliary / outreach service;
  • ensure timely payment of all fees and accounts for which you are responsible.

MonashLink has policies that everyone is required to adhere to:

  • No smoking is allowed;
  • No alcohol or illicit drugs are to be brought onto the premises or consumed on the premises.
