Allied Health Services 

At Link Health and Community 43 professionals work across three multidisciplinary teams in Allied Health Services

  • Diabetes and Chronic Disease Team
  • Home and Community Care Team
  • Childrens Services

They aim to promote independence and provide support for individuals, enabling them to make decisions about their health and wellbeing.

Services are delivered from 4 sites and include diabetes education, nutrition and dietetics counselling, disordered eating, community nursing, occupational therapy, physiotherapy, podiatry and speech pathology. They are provided as individual sessions, multidisciplinary sessions, group programs and home visits.

There are a number of programs and services that promote the health and independence of older adults, including strength training, falls prevention and planned activity groups as well as home modification and assessment. Children’s services are offered from the Oakleigh site. A dedicated team of service coordinators ensures that the client journey across the health service is holistic, integrated and coordinated.
