Link Health and Community provides services to people who have or who are at risk of having poor health. Our services are for everyone, not just those who live in the City of Monash.
How can I access a service?

What will it cost?
Link Health and Community is a non-profit organisation receiving funds from a variety of government programs. Link Health and Community is required by the Victorian Department of Human Services to charge a small fee for some services, depending on circumstances.
Primary Care fees range from $10 per session for Concession card holders and those on low incomes to $15 per session for those on a medium income, and $95 per hour for those on a high income. The fees are in line with State and Commonwealth guidelines for Home and Community Care and Primary Health Services.
Counselling and Addiction Recovery services are usually free of charge.
When you call for an appointment the Intake and Referral Officer will advise you of the fees payable.
