As a consumer of health services, you will find it important to understand what your responsibilities are when receiving services from MonashLink Community Health Service.
Our health services are provided in partnership with the staff and consumers working together to achieve optimal care. To ensure this occurs you are required to:
Provide, to the best of your knowledge, relevant and accurate information that will assist your care or treatment;
Participate in your agreed health care or treatment plan;
Ask questions of relevant staff so you can make informed decisions about your health care; Back to top
Keep scheduled appointments, however in the case of cancellation, provide timely notification;
Respect the privacy, needs and dignity of others by being courteous and considerate;
Observe safety features while participating in MonashLink services / programs; Back to top
Show consideration to staff by providing a safe environment if receiving a domiciliary / outreach service;
ensure timely payment of all fees and accounts for which you are responsible.
No smoking is allowed;
No alcohol or illicit drugs are to be brought onto the premises or consumed on the premises.
