The Community Health Nurse has a central role in providing health care to our clients, especially for those with multiple health problems.
The Community Health Nurse has a variety of roles.
These may include:
• Provision of health information – either for general health and wellbeing or relating to specific medical conditions
• Thorough assessment of health care needs
• Assessment for government funded personal alarms
• Working with others involved in your health care
• Referral – either to other MonashLink services or to external agencies to further address health needs
• QUIT smoking counselling
